** Use all forms above to register for summer camps. You do not need to fill out Photo and Medical Release if you are already a student of CMT.
Registration will be held at the
Ormond Beach Performing Arts
Center on the Registration side of the building on the
following dates:

can download the registration packets and fill them out early and
either mail them with a check to our mailing address
102 E. Granada Blvd, 2nd Floor, Ormond
Beach, FL 32173 or bring them with you on one of the
registration days.
Please
note: the City Recreation registration office is not
associated with our company and does not take any CMT registrations.
We are only available to register for our camps on our assigned
registration dates.
